The cost is in all the engineering and design and of course ensuring a door passes muster for a variety of reasons - crash testing, torsional ridgidity, systems, longitudinal forces exerted on the door etc. Then of course you have the not insignificant issue that the passenger cell must be designed and engineered to perfectly fit the said door and accommodate the structural forces that will be subjected to the door. I imagine the $50 million per door price tag incorporates some aspects of engineering and design of the relevant opening in the car's passenger cell.
